Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, coordinating discharge planning and ongoing outpatient care under supervision of an RN or MD. You will educate, coach, and support treatment decisions while partnering with a Care Advocate and Social Worker.

This role requires an active RN license in the state of Indiana, CPR certification, at least one year of RN experience, and a valid driver's license with reliable transportation.

Qualifications

• Active RN license specific to the state(s) assigned.
• Current CPR certification.
• 1+ year of clinical RN experience.
• Valid driver's license with reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health through teaching and supportive self-care techniques.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to clinical manager and care team.
• Plans and provides staff education.
• Participates in on-call responsibilities and ongoing education.
