Formación

Responsabilidades

• Coordinates patient care across the continuum of care.
• Mobilizes internal and external resources to facilitate case management.
• Documents patient plan of care and progress according to guidelines.
• Performs nursing assessments within 24–48 hours of admission.
• Identifies barriers to care; facilitates timely discharge.
• Participates in patient care conferences and collaborates with medical staff.
• Monitors resource utilization and length of stay.
• Assesses and coordinates transitions of care and post-discharge planning.
