Job summary

Providence St. Vincent Medical Center is seeking a Care Manager RN to join the Care Management team on a full-time daytime schedule.

The role focuses on screening post-acute needs, transition planning, care coordination, and education to support patients through the continuum of care. Required qualifications include an RN license in Oregon, a nursing degree, and 2 years of acute care or community-based experience, plus EMR experience.

Qualifications

Nursing degree/diploma required upon hire.
Bachelor's degree in Nursing or higher within 3 years of hire.
Oregon Registered Nurse License upon hire.
Two (2) years acute care hospital or community based experience.
One (1) year experience with electronic medical records or documentation systems.
One (1) year home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience.

Responsibilities

Screen for post-acute needs; transition planning; care coordination; education; community care referrals; care planning per regulatory requirements; collaboration with continuum care team; ongoing monitoring of plan effectiveness and reassessment of CM needs.
Provide patient-focused care management in the acute care setting and support progression through the continuum of care.
