Job summary

Providence Portland Medical Center is seeking a Care Manager RN - Social Work to support patient transitions and care coordination in the acute care setting. Multiple FTEs and shifts are available with both part-time and full-time options.

The role requires an RN with a nursing degree, Oregon license, and 2 years of acute care or equivalent experience, plus EMR familiarity. Providence offers strong benefits and retirement plans.

Qualifications

Nursing degree/diploma required upon hire.
Bachelor's degree in Nursing or higher within 3 years of hire.
Oregon Registered Nurse License upon hire.
2 years of acute care hospital or community-based experience.
1 year of experience with electronic medical records or documentation systems.
1 year of home health, mental health, substance use, hospice, utilization review or care management experience.

Responsibilities

Screening for post-acute needs and transition planning.
Care coordination and education across the continuum of care.
Collaboration with the care team and community referrals.
Ongoing monitoring and reassessment of care plans per regulatory requirements.
