Job summary

Wellstar Cobb Hospital is hiring an RN Care Coordinator for a full-time day shift. The role focuses on assessing transitional care needs, coordinating across the continuum, and engaging patients and families to ensure care goals are met.

You will collaborate with physicians and the care team to plan and monitor discharge, address barriers, and document progress in the EMR. Requires an Associate's Degree in Nursing, Georgia RN license, and at least 1 year of acute care experience.

Qualifications

• Associate's Degree in Nursing from an accredited school; GA RN license required.
• Minimum 1 year nursing experience in acute care setting.
• Excellent written and verbal communication; ability to work under stress in fast-paced environments.

Responsibilities

• Assess transitional care needs and coordinate across the continuum.
• Engage patient and family to ensure care needs are met and discharge planning.
• Collaborate with physicians and care teams to promote timely discharge.
• Document assessments and discharge plans in the medical record.
• Participate in interdisciplinary rounds and care conferences.
