Trinity Health in Minot, North Dakota, is seeking a licensed Physical Therapist to provide comprehensive PT services in an acute care hospital.

You will evaluate patients, develop individualized treatment plans to restore mobility and independence, collaborate with physicians and the care team, support safe discharge planning, educate patients and families, and document progress in the EMR.

Formación

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on physician referrals and clinical findings.
• Implement interventions to improve strength, mobility, balance, endurance, and functional performance in medically complex patients.
• Monitor patient response to treatment and modify care plans as appropriate to ensure safe, effective progression of therapy.
• Collaborate with physicians, nursing staff, case management, and other rehabilitation professionals to support coordinated care and discharge planning.
• Educate patients and family members on mobility techniques, safety precautions, equipment use, and post discharge exercise programs.
• Document evaluations, treatment sessions, and progress notes accurately and timely within the electronic medical record in accordance with regulatory and organizational standards.
