Job summary

Trinity Health seeks a Physical Therapist to provide comprehensive PT services in an acute care hospital setting. You will evaluate, develop, and implement individualized treatment plans to restore mobility and functional independence, supporting safe discharge planning and optimal patient outcomes.

Collaborate with physicians, nursing, and rehabilitation teams to ensure safe progression and regulatory compliance. Documentation and patient education are essential components of care.

Qualifications

• Current PT license in the state of North Dakota.
• DPT preferred; relevant clinical experience in acute care.
• Ability to collaborate with interdisciplinary teams.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on physician referrals and clinical findings.
• Implement therapeutic interventions to improve strength, mobility, balance, endurance, and functional performance in medically complex patients.
• Monitor patient response to treatment and modify care plans as appropriate to ensure safe, effective progression of therapy.
• Collaborate with physicians, nursing staff, case management, and other rehabilitation professionals to support coordinated care and discharge planning.
• Educate patients and family members on mobility techniques, safety precautions, equipment use, and post discharge exercise programs.
• Document evaluations, treatment sessions, and progress notes accurately and timely within the electronic medical record in accordance with regulatory and organizational standards.
