Job summary

Advocate Health is seeking a licensed Physical Therapist for PRN opportunities across our acute care locations. This on-site role will be considered for placement at Atrium Health Cabarrus, Mercy, or CMC Main based on need.

You will evaluate patients, develop care plans, coordinate with physicians and staff, and educate patients and families on home exercise programs. Supervisory duties include PT Assistant and student supervision as needed.

Qualifications

• Licensed Physical Therapist in the applicable state.
• CPR Certification and BLS as required by policy guidelines.

Responsibilities

• Evaluates patients and establishes a care plan including modalities, exercises, and patient education.
• Gathers and interprets data from medical records and family to support patient care.
• Documents evaluations, tests, progress notes, discharge notes, and charges.
• Re-evaluates patients and modifies goals as needed.
• Communicates progress to physicians and care team and coordinates schedules and discharge planning.
• Provides patient and family education with home exercise instructions and equipment use.
• Directs and supervises PT Assistant, PT Technician, students, and volunteers, and assists with orientation.
