Job summary

Foundation Health Partners seeks a licensed Physical Therapist to evaluate patients, develop and implement treatment plans, and monitor progress in an acute care hospital setting in Alaska.

The role requires completion of an APTA-accredited program, current licensure, and BLS. Weekend shifts (8-hour) on Saturdays and Sundays are available, with opportunities to work with a multidisciplinary team and coordinate discharge planning.

Qualifications

• Graduated from an APTA-accredited school of Physical Therapy.
• Licensed to practice Physical Therapy in the state of practice; BLS certification required.

Responsibilities

• Evaluate patients and develop plan of care with measurable goals.
• Implement individualized treatment plans and document progress.
• Educate patients, families, and staff; coordinate with the interdisciplinary team.
• Communicate treatment results with physicians, nurses, and care team.
• Ensure compliance with policies, regulatory requirements and professional standards.
• Supervise and delegate tasks to therapy staff as needed.
• Attend department meetings and support process improvement.
