• Evaluates and treats patients in the acute care setting to determine functional limitations and rehabilitation needs.
• Develops individualized treatment plans based on patient evaluations and goals.
• Monitors progress and modifies plans to maximize outcomes.
• Educates patients and caregivers on exercises, home programs, and safety strategies.
• Collaborates with physicians and interdisciplinary team to coordinate care.
• Maintains accurate documentation per standards.
