Qualifications

• Previous experience in an acute care, transitional care, rehabilitation, or hospital setting preferred.
• Strong clinical assessment, documentation, communication, and interdisciplinary collaboration skills.
• Ability to provide safe, effective, evidence-based physical therapy services while complying with standards.

Responsibilities

• Evaluates patients and implements individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Delivers skilled PT interventions aligned with care plans and adjusts based on patient progress.
• Educates patients, families, and care teams to support rehabilitation goals.
• Collaborates with physicians, nurses, case managers, and rehab colleagues to optimize outcomes.
• Documents evaluations, treatments, progress notes, and billing accurately per policy.
