Job summary

Qualifications

• Licensed CA State Board Physical Therapist.
• Current BLS certification.
• Ability to document evaluations, progress notes and discharge summaries in compliance with hospital policies.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide PT interventions including exercises, balance training, gait training and activities.
• Educate patients, families and caregivers on treatment plans and home exercises.
• Collaborate with physicians, nurses, case managers and other health professionals to coordinate care and discharge planning.
• Document evaluations, progress notes and discharge summaries accurately and timely.
• Maintain compliance with hospital policies, infection control protocols and professional standards.
