Job summary

Advocate Trinity Hospital in Chicago seeks a licensed Physical Therapist for a part-time role in Acute Care Rehabilitation. You will evaluate patients, develop care plans, and implement treatments to meet functional goals, collaborating with physicians and the care team.

Maintain weekend availability (at least 1X/month) and cover weekend day shifts; you will document progress, communicate changes to the team, and ensure safe operation of modalities and equipment in accordance with policy.

Qualifications

• Doctorate degree in Physical Therapy from an accredited program.
• Licensed Physical Therapist in the state of practice.
• BLS for Healthcare Providers as required by department.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Plans for and completes patient discharge with home programs and family teaching.
• Documents evaluations, progress notes, and discharge summaries per policy and standards.
• Maintains productivity standards and adheres to billing and documentation policies.
• Promotes patient safety and participates in infection control measures.
• May supervise PT assistants and rehab aides as applicable.
