Job summary

Advocate Condell Medical Center in Libertyville, IL is seeking a Part-time Physical Therapist for acute care rehabilitation. The role requires weekend work at least once a month, two late shifts per month, and at least one holiday per year.

You will interpret referrals, perform comprehensive evaluations, and develop an individualized care plan, monitoring progress and updating physicians. Documentation, billing adherence, and safety compliance are required throughout patient care.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements treatment based on evaluation and goals; monitors outcomes and adjusts as needed.
• Plans for discharge including home programs, teaching, referrals, and equipment as indicated.
• Maintains documentation per policy, including evaluations, progress notes, and discharge summaries.
• Ensures compliance with billing, scheduling, and site productivity standards; participates in safety/infection control measures.
• Supervises trainees or rehab staff as requested and supports ongoing professional development.
