Job summary

UCSF Health is seeking a per diem Inpatient/Acute Care Services Pharmacist to provide pharmaceutical care across UCSF Health locations. The role includes reviewing medication histories, assessing profiles, and delivering drug information to patients and clinicians to ensure safe and effective therapy.

The incumbent will educate patients on proper medication use, collaborate with physicians on therapy plans, and support a secure distribution system.

Qualifications

• Graduated from an accredited school of pharmacy with PharmD or equivalent as determined by the National Board of Pharmacy.
• Current California pharmacist license in good standing.
• Completion of ASHP-accredited PGY1 Pharmacy Residency, or at least 3 years inpatient/acute care pharmacy experience.
• Advanced practice board certification (e.g., BPS, AAHIVP) preferred.
• Experience participating in pediatric and adult emergency situations.

Responsibilities

• Obtain medication histories and review/assess medication profiles.
• Provide drug information to patients and health care providers.
• Collaborate with clinicians on drugs of choice and therapeutic plans.
• Ensure access to medications and educate patients on proper use.
