Job summary

UCSF Health seeks an Inpatient/Acute Care Services Per Diem Pharmacist to deliver comprehensive pharmaceutical care to patients across UCSF Health locations. You will obtain medication histories, review therapies, and provide drug information to clinicians and patients, contributing to safe, effective treatment plans.

Under supervision, you will support distribution processes, educate patients, and collaborate with pharmacists, clinicians, and students.

Qualifications

Must have graduated from an accredited school of pharmacy with a PharmD or equivalent.
For graduates in a non-US country, must have FPGEC Certification.
Must possess a current license in good standing to practice pharmacy in the State of California.
Current Basic Life Support (BLS) certification from the American Heart Association.

Responsibilities

Provide pharmaceutical care to inpatients across UCSF Health locations; ensure safe and effective drug therapy.
Obtain medication histories and review/assess medication profiles.
Provide drug information to patients and health care teams; collaborate on therapy plans.
Educate patients on proper medication use and potential adverse effects.
Assist in pharmaceutical distribution activities and support technicians and learners as needed.
