Job summary

Northwell Health in New York seeks a Physician Assistant / Nurse Practitioner to support patient admission, assessment, diagnosis, treatment, and follow-up. You will participate in rounds, order and interpret tests, and coordinate discharge planning, working closely with physicians and the care team.

Candidates must hold a Bachelor’s degree, an active New York NP license, NCCPA certification, BLS, and a Master’s in a Nurse Practitioner program; RN license in NY is also required.

Qualifications

• Bachelor’s degree from an accredited program is required.
• Master’s degree in a Nurse Practitioner program is required.
• Current NY State NP license is required.
• Current NCCPA certification is required.
• Current BLS certification is required.
• Current NY State RN license is required.

Responsibilities

• Takes initial patient histories and performs physical examinations.
• Initiates, documents, and communicates the plan of care and follows up for screening and preparation.
• Performs daily patient rounds.
• Writes orders for medications, labs, and diagnostics.
• Interprets laboratory and test results.
• Collaborates with physicians and care teams to ensure quality care.
• Notes discharge summaries, prescriptions, and referrals on charts at discharge.
• Informs patients of post-discharge care instructions.
