Summary:

The Hospital of the University of Pennsylvania seeks an experienced Nurse Practitioner (NP) to join the inpatient Palliative Care Program. Palliative Care specialists provide pain and symptom management, psychosocial support, and advance care planning for patients and families facing serious illness. The inpatient palliative care clinician works as a consultant throughout the hospital, directly engaging with patients and families and offering recommendations to the primary healthcare team.

Our interdisciplinary team collaborates to provide patient- and family-centered palliative care focused on psychosocial, spiritual, and medical needs. Consultations cover all acute care settings, including oncology floors, general medical-surgical floors, specialty units, and critical care units.

The NP will provide expert symptom management and facilitate crucial discussions with patients and families to develop personalized, comprehensive care plans aligned with patient goals. The NP will also participate in daily inter-professional palliative care team rounds to support collaboration and communication.

All CRNPs function under the guidelines established by the University of Pennsylvania Medical Center and are credentialed through that body. The CRNP will be accountable via a formal job description, delineation of privileges, and agreement with their collaborating physician. Clinical work hours and requirements are defined and managed under the direction of the collaborating physician and within Human Resource guidelines. The Nurse Practitioner contributes to excellence in education, leadership, professional contribution, and practice.

Responsibilities:

  1. Provides direct clinical care according to established protocols, utilizing evidence-based standards in collaboration with the team.
  2. Maintains accurate and confidential medical records, documenting evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations in the electronic health record (EHR). Properly identifies self to patients and families, clarifying the NP role if necessary.
  3. Functions within their scope, referring to the collaborating physician when outside of scope or as directed.
  4. Performs interventions specific to the patient population and collaborative agreement.
  5. Participates in quality improvement processes to meet care and service standards.
  6. Demonstrates professional responsibility as a role model to peers and the healthcare team.
  7. Performs approved therapeutic or diagnostic procedures based on patient status.
  8. Addresses concerns of patients and families, building therapeutic relationships for support.
  9. Provides patient education, counseling, and instructions regarding diagnosis, treatments, medications, and prevention.
  10. Supports accreditation, compliance, and regulatory activities.
  11. Precepts NP students and other learners.
  12. Identifies patient/family needs for community resources and refers appropriately.

Minimum Requirements:

  1. Master’s prepared Nurse Practitioner with Acute Care certification.
  2. Current Pennsylvania license (or eligible to obtain).
  3. 2-3 years of experience as an Advanced Practice Provider in acute care.
  4. Training/experience in palliative care, hospice, or geriatrics preferred.
  5. Strong communication skills and ability to work independently across various medical and surgical services.

Credentials:

  1. BLS/CPR as per American Heart Association requirements.
  2. PA and/or NJ CRNP licensure and Prescriptive Authority.
  3. PA and/or NJ RN licensure and National Board Certification.

Education or Equivalent Experience:

  1. Master of Arts or Science (Required).
  2. Master's Degree or DNP in Nursing or equivalent experience. Newly licensed or <2.5 years requires review and approval by the Credentials Committee.
