Qualifications

Position Details

The lung transplant advanced practice provider (APP) manages patient care throughout the post-transplant and ongoing phases. The APP plays a key role in the transplant team, ensuring continuity and quality of care. This role offers an opportunity to provide longitudinal care to a complex patient population within a multidisciplinary team.

Schedule and Requirements
  • Schedule: Monday-Friday, 8:00 AM - 5:30/6:00 PM; no standard holidays
  • Occasional on-call and weekend duties
Responsibilities
  • Provide direct patient care based on protocols and evidence-based practices in collaboration with the team.
  • Maintain accurate, confidential medical records in the electronic health record (EHR).
  • Introduce yourself to patients and families, clarifying your role as an APP.
  • Work within your scope of practice, referring to physicians when outside your scope or as directed.
  • Perform interventions specific to the transplant patient population.
  • Participate in quality improvement initiatives.
  • Serve as a role model professionally.
  • Perform diagnostic and therapeutic procedures as appropriate.
  • Address concerns of patients and families, providing support and education.
  • Engage in accreditation, compliance, and regulatory activities.
  • Precept students and learners.
  • Identify and refer patients to community resources as needed.
Qualifications
  • Acute Care Nurse Practitioner or Master's Physician Assistant (required)
  • Master's Degree (required)
  • BLS/ACLS certification (required)
  • Current licensure or eligibility in Pennsylvania, with credentialing eligibility
  • Transplant experience (preferred)
  • New graduates considered on an individual basis
