Position: Lung Transplant Advanced Practice Provider (APP)

The lung transplant APP provides collaborative management of patient care throughout the post-transplant and longitudinal phases. The APP serves as an integral member of the transplant team, ensuring continuity of care and meeting the needs of transplant patients. This role offers an excellent opportunity to provide longitudinal care to a complex population within a multidisciplinary team.

Schedule: M-F 8-5:30/6PM; no standard holidays

Occasional on-call and weekend requirements

Responsibilities:
  1. Provides direct clinical care for the patient population according to established protocols, utilizing evidence-based standards in collaboration with the team.
  2. Maintains accurate and confidential medical records, documenting evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations in the electronic health record (EHR).
  3. Identifies self properly to patients and families, clarifying the APP role if necessary.
  4. Functions within scope and refers to the collaborating physician when outside scope or as directed.
  5. Performs interventions specific to the patient population and collaborative agreement.
  6. Participates in performance improvement processes to meet quality standards.
  7. Demonstrates professional responsibility as a role model to peers and the healthcare team.
  8. Performs approved therapeutic or diagnostic procedures based on patient status.
  9. Addresses patient and family concerns, fostering a therapeutic relationship.
  10. Provides patient education, counseling, and instruction regarding diagnosis, treatments, medications, and prevention.
  11. Supports accreditation, compliance, and regulatory activities.
  12. Precepts students and healthcare learners.
  13. Identifies community resource needs and refers appropriately.
Credentials:
  • Acute Care Nurse Practitioner or Master's Prepared Physician Assistant (required)
  • Master's Degree (required)
  • BLS/ ACLS certification (required)
  • Current licensure or eligibility in PA and credentialing eligibility
  • Transplant experience (preferred)
  • New graduates considered on an individual basis
