Qualifications

• Bachelor's Degree in nursing preferred with minimum of 3 years clinical experience.
• 3-5 years of direct patient care or case management experience in a similar size hospital.
• Current license as a Registered Nurse in Florida. BLS from AHA; ACLS preferred.

Responsibilities

• Coordinate and manage care to meet patients' needs and ensure handover and continuity of services.
• Analyze patient status, treatment plan and history to identify gaps in physician documentation.
• Consult with medical staff on status, payer requirements and resource utilization.
• Assure appropriate sequencing of care/interventions for optimal results and smooth transitions.
• Identify and remove barriers that impede progress of care and coordinate admission status decisions.
• Maintain knowledge of health care economics, reimbursement methodologies and apply daily.
