Holy Cross Hospital in Fort Lauderdale, Florida is seeking a full-time Case Management Registered Nurse to coordinate and manage patient care across the acute, ambulatory, and post-acute continuum. The role emphasizes handover, continuity of services, and collaboration with interprofessional teams to improve communication across care settings.

The position requires a Florida RN license, 3+ years of direct patient care or case management experience, and BLS certification; ACLS preferred.

Formación

• Bachelor's degree in nursing preferred with 3+ years clinical or case management experience.
• Active Florida RN license required; BLS certification; ACLS preferred.

Responsabilidades

• Ensures appropriate clinical documentation to support level of care and identifies gaps in documentation.
• Serves as consultant and educator to medical staff on status, placement, payer requirements, and utilization of resources.
• Assures appropriate sequencing of care for optimal results and smooth transition along hospital care continuum.
• Identifies and removes barriers that impede progress of care and coordinates admission status determination.
• Maintains knowledge of healthcare economics, trends, and reimbursement methodologies for daily practice.
