Holy Cross Health Fl in Fort Lauderdale is seeking an Acute Care Manager to coordinate and manage care, ensuring handover and continuity across the continuum. The role collaborates with acute, ambulatory, and post-acute teams to improve communication and reduce fragmentation of care.

The position is part-time, on-site per diem. Requirements include a Bachelor's in Nursing (preferred) with 3+ years of clinical or case management experience; Florida RN license; BLS; ACLS preferred.

Qualifications

• Bachelor's Degree in nursing preferred with a minimum of 3 years clinical experience.
• Experience: 3-5 years of direct patient care or case management in a hospital setting preferred.
• Acting independently, analyze clinical, financial and social situations and direct the healthcare team with urgency.

Responsabilités

• Coordinate and manage care to meet patients' multiple needs and ensure handover across the continuum.
• Collaborate with interprofessional acute, ambulatory and post-acute care teams to improve communication.
• Identify barriers to progress of care and coordinate level of care with admission status.
• Ensure appropriate clinical documentation to support level of care and identify gaps in documentation.
• Educate medical staff on status, payer requirements and resource utilization.
