Job summary

Holy Cross Hospital in Fort Lauderdale, FL is seeking an Acute Care Manager to coordinate and manage care across the hospital's acute, ambulatory and post-acute teams, ensuring handover and continuity of services.

This on-site, part-time role requires a registered nurse with 3+ years of clinical or case management experience, strong analytical skills, and the ability to educate medical staff and remove barriers to care.

Qualifications

• Bachelor's degree in nursing preferred with at least 3 years of clinical experience.
• 3–5 years of direct patient care or case management experience.
• Ability to analyze clinical and social situations, with urgency and leadership.

Responsibilities

• Participates in Performance Projects to be determined by Manager/Director.
• Ensures appropriate clinical documentation to support level of care.
• Serves as consultant and educator with medical staff regarding status, placement, payer requirements, and utilization of resources.
• Assures appropriate sequencing of care for optimal results and smooth transition along hospital care continuum.
• Identifies and removes barriers that impede the progress of care and coordinates admission status accordingly.
• Maintains knowledge of healthcare economics and applies to daily practice.
