SummitStone Health Partners is seeking a Registered Nurse-Outpatient in Fort Collins, CO for a full-time role. The RN will support collaborative treatment planning, monitor vital signs, administer medications and treatments, perform lab tests, and coordinate transitions of care in behavioral health settings.

Candidates must hold a Colorado RN license in good standing; a new grad with interest in behavioral health and withdrawal management may be considered.

Formación

• Unencumbered Colorado license as a registered nurse.
• Colorado license or Compact license in good standing.
• Registered Nurse License required.
• Valid Colorado driver’s license or ability to obtain within 30 days of hire and a clean driving record required.
• DORA license or ability to obtain within 90 days of hire required.
• Basic Life Support Certification, or ability to obtain within 30 days of hire.

Responsabilidades

• Facilitate and implement collaborative treatment planning, including monitoring vital signs, medication and treatment administration.
• Perform laboratory testing and ongoing physical and behavioral health assessments.
• Engage with clients and families, coordinating transitions of care.
• Ensure compliance with all rules, guidelines, and directives.
