Job summary

This on-site, per diem, part-time role requires an RN with a Bachelor’s degree preferred and 3–5 years of direct patient care or case management.

Qualifications

• Bachelor's degree in nursing preferred with 3+ years clinical experience.
• 3-5 years of direct patient care or case management experience in a similar size, similar scope hospital preferred.
• Acting independently, must demonstrate an ability to analyze clinical, financial, and social situations, identifying problems and directing the healthcare team.

Responsibilities

• Coordinate and manage care to assist patients in meeting their multiple needs, ensure handover and continuity of services.
• Collaborate with interprofessional acute care, ambulatory and post-acute care management teams to improve communication across the continuum of care.
• Identify and remove barriers that impede progress of care and coordinate level of care with admission status determination.
• Support accurate documentation and ensure appropriate sequencing of care/interventions for optimal results.
