Responsibilities

• Assess and coordinate patient care across the continuum using accepted criteria to manage benefits and determine medical necessity.
• Achieve optimal clinical and quality outcomes by managing care and resources to reduce unnecessary utilization.
• RN Case Manager reports to the Director of Case Management or designee.
