Formación

• RN with an Associate's degree in nursing or higher.
• 2+ years of acute hospital-based nursing experience is preferred.
• Experience in acute care hospital case management is preferred.
• CCM/ACM certification is preferred.

Responsabilidades

• Assess and coordinate patient care across the continuum.
• Utilize criteria and guidelines to manage benefits and determine medical necessity.
• Support optimization of clinical outcomes and reduce unnecessary utilization.
