Hillcrest Hospital South in Tulsa, OK is seeking a Day shift, full-time Case Management Acute Care Case Manager RN. You will assess and coordinate patient care across the continuum using guidelines to manage benefits and determine medical necessity.

The RN Case Manager reports to the Director of Case Management. Requirements include an RN with an associate degree or higher and BLS within 14 days; two years of acute hospital nursing experience are preferred, with CCM/ACM certification as a plus.

Qualifications

• BLS certification must be obtained within 14 days of hire or transfer.

• The Case Manager RN is responsible for assessing and coordinating patient care across the continuum, utilizing accepted criteria and guidelines to manage benefits and determine the medical necessity of the services provided.
• This position functions to achieve optimal clinical and quality outcomes by effectively managing care and resources to reduce unnecessary utilization.
• The RN Case Manager reports to the Director of Case Management or designee.

Job Requirements:

• RN, associate's degree in nursing or higher
• BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care

Preferred Job Requirements:

• Two (2) years acute hospital-based nursing experience
• Certification in case management (ACM or CCM)
• Experience in acute care hospital case management
