Southern NH Medical Centers is seeking an Acute Care Case Manager to guide discharge planning, assess patient needs, and coordinate resources for a smooth transition from hospital to home or another care setting.

The role emphasizes multidisciplinary collaboration, utilization review, and care coordination with families and clinical teams, with a focus on culturally appropriate, age-specific planning.

Qualifications

• Bachelor's degree in a healthcare-related field or equivalent experience.
• ASN/BSN preferred but not required.
• 3–5 years of case management-related health care experience.
• A minimum of 7 years of experience is required without a bachelor’s degree or a nursing degree.
• Experience with patient documentation in an EMR system preferred.

Responsabilités

• Assess patients’ needs during the discharge planning process.
• Provide resources and community referrals for continuity of care.
• Coordinate care with families, providers, and the clinical team.
• Conduct Utilization Reviews.
