Job summary

Southern NH Medical Centers seeks an Acute Care Case Manager to assess patients’ health status and coordinate discharge planning. You will develop multidisciplinary, culturally appropriate care plans and complete clinical reviews to determine appropriate level of care and insurance coverage.

The role requires a Bachelor’s in a healthcare field or equivalent, with 3–5 years of case management experience; ASN/BSN is preferred but not required.

Qualifications

• Bachelor's degree in a healthcare-related field or equivalent experience.
• ASN/BSN preferred but not required.
• 3 – 5 years of case management-related health care experience.
• A minimum of 7 years of experience is required without a bachelor’s degree or a nursing degree.
• Experience with patient documentation in an EMR system preferred
