Job summary

NHC HealthCare Lexington seeks an Activity Assistant II to plan and provide meaningful activities for residents at 2993 Sunset Blvd, West Columbia, SC. You will design programs that meet individual needs and create an atmosphere of fun and engagement for seniors.

The role requires creativity, energy and knowledge of wellness domains in long-term care, with responsibilities including assessment, documentation, and collaboration with families and staff.

Qualifications

• MDS 3.0, care planning and documentation experience required.
• Ability to work with family members, partners and the community.
• High School Diploma or GED required.
• Qualified recreation specialist or two years in a social/recreational program in health care.

Responsibilities

• Assess residents to determine activity interests and needs.
• Provide therapeutic activities meeting individual interests and needs.
• Provide activities addressing physical, social, mental, emotional, and spiritual well-being.
• Maintain all required records.
• Promote center services to local clubs, businesses and schools.
• Attend center meetings as required.
• Follow NHC standards and Better Way promises.
• Adhere to state, federal, company and center policies, including HIPAA.

Activity Assistant II at NHC HealthCare Lexington

NHC Healthcare Lexington, located at 2993 Sunset Blvd. in West Columbia, SC is looking for an Activity Assistant II to join our fun team and assist in providing therapeutic programs which meets each patient's "individual" needs while providing an atmosphere of FUN and ENJOYMENT and other duties as requested. Applicant must have a desire to work with seniors, be compassionate, caring, and creative. The ideal candidate must be creative, energetic, and have knowledge of the domains of wellness as it relates to programming in long term care. Your goal would be to provide daily meaningful activities that enhance the quality of life for our residents.

Specifically, You Are Responsible For

• Assessing patients to determine activity interest and needs
• Provide therapeutic activities which meet the individual interest and needs
• Provide activities which meet the physical, social, mental, emotional, and/or spiritual needs of patients while providing an atmosphere of fun and enjoyment
• Maintain all required records
• Maintain good public relations with the community by contacting and informing local clubs, businesses, schools, etc. of center services and goals
• Attending center meetings as required
• Be aware of the NHC standards and abide by the Better Way promises
• Be aware of and follow all state, federal, company and center policies and procedures
• Be aware of and follow HIPAA regulations

Other Important Skills Are

• Sense of humor with the ability to laugh at success and failure
• Must be self-motivated, creative with a positive attitude and genuine enthusiasm with a willingness to learn innovative ideas and skills
• Possess patience and good organizational skills, assertiveness and be a conscientious person

• Flexibility in working hours so that weekend or evening events may be provided.

Requirements

• Must have experience in MDS 3.0, care planning and documentation
• Must be able to work with family members, partners, and the community
• Must have High School Diploma or GED.
• Qualified recreation specialist for activities professional who is eligible for certification by a recognized body or two years’ experience in a social or recreational program with the last five years (one of which was full‑time in a patient activities program in a health care setting), OR has completed a training course approved by the state
