Sonoma Post Acute is seeking an energetic Activities Director to lead our resident activities and enrichment program in Sonoma, CA. You will develop, implement, and evaluate a comprehensive plan to promote physical, emotional, social, and cognitive well-being for residents.

This full-time role requires collaboration with nursing, therapy, and dietary teams, scheduling engaging events, documenting progress, and ensuring regulatory compliance.

Qualifications

  • High school diploma or equivalent required.
  • Previous experience in activities, recreation, senior living, or healthcare preferred.
  • Strong organizational and event-planning skills.
  • Excellent communication and interpersonal abilities.
  • Ability to lead group activities and engage diverse populations.

Responsabilités

  • Plan, organize, and direct daily activity programs for residents.
  • Develop monthly activity calendars with engaging events.
  • Coordinate group activities, special events, and community outings.
  • Evaluate resident participation and adjust programs accordingly.
  • Recruit, train, and supervise activity staff and volunteers.
  • Complete activity assessments, care plans, and required documentation.

This position works closely with residents, families, nursing staff, and interdisciplinary team members to enhance resident engagement and overall quality of life.Essential ResponsibilitiesProgram Development & ManagementPlan, organize, and direct daily activity programs for residents.Develop monthly activity calendars that provide a variety of engaging and meaningful events.Create programs that address residents' physical, emotional, cognitive, social, and spiritual needs.Coordinate group activities, special events, entertainment, holiday celebrations, and community outings.Evaluate resident participation and modify programs based on resident interests and abilities.Resident EngagementConduct resident assessments to identify interests, hobbies, preferences, and activity needs.Encourage participation and foster positive relationships with residents and families.Provide one-on-one visits and individualized activities for residents unable to attend group programs.Promote resident independence, dignity, and quality of life.Leadership & CoordinationRecruit, train, supervise, and support activity staff and volunteers.Coordinate with department leaders to enhance resident programming.Collaborate with nursing, therapy, social services, and dietary teams to support resident-centered care.Assist with resident council meetings and family engagement initiatives.Documentation & ComplianceComplete activity assessments, care plans, progress notes, and required documentation.Ensure compliance with state, federal, and facility regulations.Maintain accurate records related to resident participation and program effectiveness.Participate in quality assurance and performance improvement activities.QualificationsRequiredHigh school diploma or equivalent.Previous experience in activities, recreation, senior living, healthcare, skilled nursing, assisted living, or a related field.Strong organizational and event-planning skills.Excellent communication and interpersonal abilities.Ability to lead group activities and engage diverse resident populations.Compassionate, energetic, and creative approach to resident programming.PreferredActivities Director Certification (ADC), Certified Therapeutic Recreation Specialist (CTRS), or related credentials.Experience in a Skilled Nursing Facility (SNF) or Post-Acute setting.Experience supervising staff or volunteers.Knowledge of state and federal regulations related to activity programming.Ideal CandidateWe're looking for someone who:Loves working with seniors.Brings enthusiasm, creativity, and positive energy to the workplace.Enjoys planning events and building meaningful resident relationships.Has strong leadership and organizational abilities.Believes every resident deserves opportunities for engagement, purpose, and joy.About Sonoma Post AcuteAt Sonoma Post Acute, we are committed to providing exceptional skilled nursing, rehabilitation, and long-term care services in a compassionate and welcoming environment.
