Job summary

Pathways to Life, Inc. is seeking a Master’s level Qualified Professional to join an Assertive Community Treatment (ACT) team in Greenville, NC.

You will lead person-centered planning, provide a wide range of community-based rehabilitative and clinical interventions, and support members toward recovery, with opportunities to develop a specialty area such as trauma-focused care or housing advocacy. The role requires daily local travel, 1.0 FTE full-time status, and supervision by the ACT Team

Qualifications

• Master’s degree in a human services field and Qualified Professional status.
• Knowledge, skills, and abilities for serving adults with serious mental illness.
• Valid NC driver’s license and reliable transportation.
• Ability to pass background checks and health screenings.
• CPR/First Aid certification or willingness to obtain within 30 days.
• Proficiency with electronic health records and standard office software.

Responsibilities

• Facilitate person-centered planning for assigned members.
• Deliver psychosocial rehabilitative interventions in daily living and social skills.
• Provide CBT interventions within training scope.
• Provide tenancy support and advocacy for housing.
• Assist with entitlements, benefits, and redeterminations.
• Engage families and provide psychoeducation.
• Assess risk, develop crisis plans, and monitor treatment.
