Job summary

SpringPath Mental Health Services, part of Sevita, seeks a Program Assistant to support Assertive Community Treatment. You will assist with triage, scheduling, crisis assessment, and care coordination for clients with significant mental health needs.

This role emphasizes teamwork, trauma‑informed practice, and adherence to best practice interventions. On‑call rotations and flexible hours are part of the schedule to meet program needs.

Qualifications

• High school diploma or GED required.
• 21 years of age or older.

Responsibilities

• Provides interventions or support services as per the Individual Crisis/Coaching Support Plan.
• Coordinates with the ACT team and community resources.
• Treats clients with dignity and self-determination.
• Completes general office duties.
• Maintains intra-agency communication with SpringPath clients and interfacing agencies.
• Complies with Data Privacy/HIPAA requirements.
• Writes progress notes and maintains client charts.
• Provides incident analyses and documents welfare and safety activities.
• Supports program outcomes and participates in evaluations.
• Pursues supervisory oversight and professional growth.
