Job summary

Postgraduate-Center-For-Mental-Heal in New York City seeks a licensed clinician to deliver ACT-based community mental health services. The role requires a strong foundation in DSM-5-TR criteria, crisis stabilization, and collaborative teamwork.

You will conduct community visits, complete assessments, provide psychotherapy with evidence-based modalities, involve families, and participate in daily/weekly clinical meetings. On-call crisis coverage rotates with the ACT team.

Qualifications

• Master's degree required; NYS licensure preferred.
• Minimum 1 year experience with individuals with serious mental illness.
• Experience providing community-based services and crisis intervention.

Responsibilities

• Conduct daily community visits with thorough progress notes and documentation.
• Complete initial assessments, Initial Service Plans and initial Comprehensive Assessments; Wellness and Safety Plans; update Service Plan Reviews every 6 months.
• Provide individual psychotherapy using evidence-based modalities.
• Participate in daily meetings and weekly clinical discussions; incorporate family input into service plans to support relapse prevention.
• Act as advocate for consumers with family, landlords and community supports; coordinate care.
• Provide 24-hour crisis intervention on-call on a rotating basis; perform duties aligned with program goals.

JOB SCOPE:This is a clinical position requiring in-depth knowledge of mental health, DSM 5-TR diagnostic criteria, and skills in crisis intervention and stabilization. As part of the ACT you must be willing to go out into the community to provide services. This position requires clinical experience, and the desire to work as part of a team.ESSENTIAL FUNCTIONS:Conducts daily visits in the community, and subsequent progress notes and other required documentation in accordance with agency, ACT, and OMH standards that demonstrate evidence-based practicesCompletes and maintains shared assigned charts such as initial assessments including Initial Service Plans and initial Comprehensive Assessments, Wellness and Safety Plans, update Service Plan Reviews with Comprehensive assessments every 6-months using evidence-based practices and assessments required, risk assessments and other documentation as required.Completes individual psychotherapy with consumers using evidence-based treatment modalities.Participates in daily morning meeting, and informs ACT team in daily meetings and weekly clinical meetings around family issues and incorporate findings into service plansInvolves family and significant others in the consumer’s treatment with the ACT team and provide psycho-education and relapse prevention based on motivational interviewing and cognitive behavioral strategies during visits.Act as advocate and liaison for consumers in relationships with family, support systems, landlords and other community supports.Provide 24-hour crisis intervention on-call services on a rotating basisPerforms other duties, consistent with the goals and objectives of the program, as directed by Team Leader.EDUCATION AND EXPERIENCE:Master’s Degree required – MHC-LP, LMHC, LMSW, LCSW, or equivalent in licensure in NYS preferred.At least one year of experience working in human services with individuals with serious mental illness and experience in work with families.Must be outcome oriented in a fast-paced environment with deadlines.
