Southwest Network, Inc. is seeking a Housing Specialist within an Assertive Community Treatment (ACT) team in Arizona.
You will assess housing and daily living needs, develop ISPs, and coordinate housing-related services for adults in a community setting, including after-hours support and on-call coverage. The role requires a degree in behavioral health or related field (or equivalent experience) and one year of work with seriously mentally ill adults, with preference for ACT experience.

As a member of an Assertive Community Treatment (ACT) team, functions as the housing specialist with primary responsibility for the provision of housing support services in accordance with SAMHSA’s evidence-based practice of permanent supportive housing. Provides comprehensive assessment of housing, residential and activities of daily living skills and needs, and leads in service planning for adult daily living skills and housing support services. Coordinates, monitors, and provides direct services to members in a community setting. Participates in the development of individualized service plans (ISPs) and provides/coordinates services related to ISPs. Provides after-hours care, on-call coverage, and support to all members being served by the ACT team.
