Job summary

Gulf Coast Center in Brazoria and Galveston Counties seeks a Case Manager for ACT/Criminal Justice to engage justice-involved individuals in the Galveston County jail and connect them to ACT services to reduce recidivism. The role combines clinical coordination with community-based support in a mobile, recovery-oriented model.

You will manage a caseload of high-utilizers, provide crisis services, collaborate with an interdisciplinary team, document care within policy, and help clients access

Qualifications

• Requires a bachelor's degree or QMHP certification
• Minimum one year related work experience in behavioral health
• Experience with criminal justice system is preferred

Responsibilities

• Delivers services per GCC standards with direct service time targets (50% of time)
• Completes accurate documentation within 24 business hours
• Manages a discrete caseload as part of an interdisciplinary team
• Participates in treatment planning and coordination with the team
• Attends team meetings and represents program professionally
• Maintains daily contact with designated staff regarding caseloads
• Provides ACT-based self-management and clinical coordination
• Offers crisis services to clients and walk-ins as needed
• Responds to after-hours ACT client needs on a rotation basis
• Ensures monthly direct service hours and adherence to the Direct Service Agreement
• Monitors service hours to meet 80% minimums within caseload
• Follows up on ACT qualifying referrals within 2 business days
• Ensures adherence to ANSA assessments and required certifications
