Job summary

InterCommunity, Inc. is seeking an ACT Community Support Specialist to provide skill-building, engagement, and care coordination as part of the Assertive Community Treatment Team.

Services are delivered in clients' homes and various community settings. The role involves collaborating on a transdisciplinary team, linking clients to medical, social, and vocational resources, and supporting recovery through individual treatment sessions and crisis involvement when needed.

Qualifications

  • Bachelor’s degree or associate degree in health or behavioral health; equivalent experience considered.
  • Experience in a community health or behavioral health setting.
  • Experience with recovery from mental illness or substance use.
  • Proficient knowledge in linking clients to community resources.

Responsibilities

  • Participate on a transdisciplinary ACT team with daily meetings to review services.
  • Link clients to clinical, medical, social, educational, rehabilitative, vocational services.
  • Collaborate to identify required services based on client needs.
  • Support client recovery through treatment sessions within the ACT team or with external providers.
  • Complete documentation per procedures, including court letters, medical records, and scheduling.
  • Engage clients and maintain attendance with tracking documents and reports.
  • Create and facilitate groups based on client needs (office and community).
  • Assist in crisis assessments and detox admission decisions as needed.
  • Provide education and consultation to clients and families for well-being.
  • Perform related duties as required.

Summary:

The ACT Community Support Specialist provides skill building, engagement, care coordination as part of the Assertive Community Treatment Team (ACTT). These services are community based and involve being in client’s homes and other community-based locations including various inpatient settings.

Essential Duties & Responsibilities:
  • Active participation on a transdisciplinary team including daily meetings to review program operations and services of clients.
  • Assist in linking each client to the necessary clinical, medical, social, educational, rehabilitative, vocational and/or other services within the team or outside providers if necessary.
  • Collaborate within the team and the clients to identify required services by assessing the overall needs of each client.
  • Support client recovery by participating in individual treatment sessions within the ACT Team or outside providers if necessary.
  • Complete all required documentation related to treatment in accordance with established procedures including but not to limited to court letters, gathering medical records, copying, faxing, scanning, collateral contact and appointment scheduling.
  • Assertive engagement with clients and support ongoing attendance by completing all tracking documents and reports to support engagement.
  • Create and facilitate groups based on client identified needs (both office and community based).
  • Provide assistance during the assessment of individuals in crisis and intoxicated clients by helping to determine the need for crisis involvement or detox and facilitate the admission to detox facility.
  • Provide education, support and consultation to clients and the individual’s families or their support system which is directed exclusively to the well being and benefit of the individual, if needed.
  • Perform related duties as required.

*All agency staff are required to attend all mandatory department/agency meetings and trainings*

Schedule:

Monday - Friday, 8:00 AM - 4:30 PM

Requirements
Education &/Or Experience:

A bachelor’s degree or an associate degree in health or behavioral health. Equivalent experience in a community health center or behavioral health setting may be considered. Experience working in a setting attending to the needs of those in recovery from mental illness and substance use/addiction. Proficient knowledge in providing linkages with various community resources.
