Job summary

Family Service League Inc. in Bay Shore, New York, is seeking a full-time ACT Case Manager to perform home visits, provide client education, and develop treatment plans. You will connect clients to treatment facilities and coordinate with care teams in a dynamic, community-based setting.

The role requires strong organizational and communication skills, a valid NY driver’s license, and experience with the SPMI population. A comprehensive benefits package is offered with a $2,000 sign-on bonus.

Qualifications

• High school diploma or equivalent required; Bachelor's degree preferred.
• At least one year of experience with the SPMI population.
• Familiarity with DSS, SSI, Medicaid Managed Care and Housing.
• Excellent organizational and interpersonal skills.
• Strong communication skills.
• Ability to multi-task in a fast-paced environment.
• Valid New York State driver's license.

Responsibilities

• Complete Entitlement Applications.
• Accompany clients to Entitlement Offices.
• Provide money management and budgeting skills.
• Ensure active entitlements, including Medicaid and/or other benefits.
• Provide psycho-education to families and clients.
• Facilitate groups.
• Complete client assessments using relapse prevention plans.
• Provide motivational interviewing.
• Connect clients to treatment facilities.
• Correspond with managed care companies.
• Apply for benefits for clients (Medicaid, SSI/SSD, HEAP, Food Stamps, etc.).
• Provide crisis intervention.
• Cover the crisis phone.
• Participate in daily team meetings and case conferences.
• Conduct daily home visits.
• All other duties as assigned.
