Job summary

BestSelf is seeking an ACT Case Manager in Dunkirk, NY to provide rehabilitation and support services within an Adult ACT Team. You will coordinate treatment plans, assist clients with daily living and budgeting, and collaborate with the program team under clinical supervision.

The role emphasizes crisis intervention, community-based support, and liaison work with community resources. A NYS driver's license and defensiv driving requirements are part of the job.

Qualifications

• Bachelor's degree in a human service field and at least two years' experience as a case manager.
• Valid NYS drivers' license; automobile in good working order.
• Accredited NYS Defensive Driving course, or obtain within 15 days of hire.
• Maintain Active NYS Defensive Driving class every 3 years, upon expiration.

Responsibilities

• Coordinate and implement case management activities.
• Consult with Program Director and psychiatrist regarding treatment responses.
• Provide activities of daily living and mobility training.
• Provide money management and budgeting training.
• Attend Morning Meetings and participate in 24-hour on-call rotation.
• Document recipient progress and manage client charts.
