Job summary

Family Service League of Suffolk County, Inc. is seeking a full-time ACT Case Manager in Bay Shore, NY. The role includes home visits, client education, and developing treatment plans while coordinating with treatment facilities.

A $2,000 sign-on bonus is offered along with a comprehensive benefits package. The position requires a high school diploma or equivalent, with a bachelor’s degree preferred. Experience with SPMI populations and knowledge of community resources is preferred.

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree preferred.
• Experience with severe mental illness (SPMI) population preferred.
• Familiarity with community resources (DSS, SSI, Medicaid) desired.

Responsibilities

• Conduct Entitlement Applications and accompany clients to offices.
• Provide money management and budgeting skills.
• Ensure active entitlements (Medicaid etc.).
• Provide psycho-education to families/clients.
• Facilitate groups and conduct client assessments.
• Provide motivational interviewing and crisis intervention.
• Connect clients to treatment facilities and manage care communications.
• Cover crisis line and participate in daily team meetings.
