Family Service League is seeking a full-timeCase Manager for the Assertive Community Treatment (ACT) program. The Case Manager conducts home visits and provides education to clients. The Case Manager also provides treatment plans and connects clients to treatment facilities.

At least one year of experience with the severe and persistent mental illness (SPMI) population and familiarity with community resources, including DSS, SSI, Medicaid Managed Care and Housing required.

Strong communication skills, both verbal and written required.
Ability to multi-task and prioritize in a constantly changing, fast paced environment.
Valid and clean New York State Driver's License required.
This position requires sitting for long periods of time and travelling to conduct home visits. Must be able to lift up to 10 lbs.
