Job summary

NorthKey Community Care is seeking an Assertive Community Treatment Care Coordinator to join the multidisciplinary Community Integration Team. This role delivers community-based care coordination, outreach, rehabilitation support, and case management to individuals with serious mental illness and co-occurring disorders, promoting recovery and community integration.

The ACT Care Coordinator engages across homes, hospitals, courts, correctional facilities, and other community settings,

Qualifications

• A bachelor's degree from an accredited college or university is required.
• Degrees in Social Work, Psychology, Human Services, Sociology, Behavioral Science or related disciplines are strongly preferred.

Responsibilities

• Engage clients in a structured, collaborative relationship that promotes recovery, self-sufficiency, independence, and community integration.
• Provide community-based services in homes, hospitals, courts, correctional facilities, shelters, and other community settings.
• Participate as an active member of the multidisciplinary ACT treatment team.
• Coordinate medical, behavioral health, housing, employment, educational, transportation, and social support services.
• Assist clients in obtaining and maintaining benefits, insurance coverage, and community resources.
• Provide outreach services to individuals who are difficult to engage in traditional clinic-based treatment.
• Maintain frequent face-to-face contact with clients in community settings.
• Support clients in developing daily living, independent living, social, vocational, and community functioning skills.
