Job summary

NorthKey Community Care is seeking an Assisted Outpatient (AOT) Care Coordinator to join the Community Integration Team. The role focuses on community-based care coordination, outreach, rehabilitation support, and case management for individuals with serious mental illness and co-occurring disorders.

Work involves varied locations across community-based settings, with a comprehensive benefits package and potential for an annual bonus.

Qualifications

• Bachelor's degree required from an accredited college/university.
• At least one year of experience in behavioral health, substance use treatment, case management, rehabilitation services, or community support services.

Responsibilities

• Engage clients in a collaborative relationship to promote recovery and independence.
• Provide community-based services in homes, hospitals, courts, and other community settings.
• Participate as an active member of the multidisciplinary AOT treatment team.
• Coordinate medical, behavioral health, housing, employment, and social support services.
• Assist clients in obtaining benefits, insurance coverage, and community resources.
• Provide outreach to hard-to-engage clients and maintain frequent face-to-face contact.
