Qualifications

• Bachelor's degree in relevant behavioral health field.
• Associate's degree with 1 year full-time behavioral health experience.
• High school diploma or GED with 3 years behavioral health experience.
• Valid AZ driver's license and proof of insurance.
• At least 21 yrs of age.
• Current CPR and First Aid certification.

Responsibilities

• Oversee clinical expertise and care oversight across ACT team.
• Assess clients with strengths-based and risk assessments.
• Engage clients/families through in-person and phone contacts.
• Coordinate care within treatment teams and with external resources.
• Complete and review progress notes and BHSP updates.
• Develop safety/crisis plans and triage emergencies as needed.
• Participate in Behavioral Health Service Plan development and updates.
• Coordinate care activities with ACT staff and services.
