Job summary

Focus Home Health San Jose is seeking a driven Account Executive to grow our referral network and establish trusted partnerships with physicians, hospitals, SNFs, and community providers in the San Jose area. This role is ideal for someone who enjoys face-to-face relationship building and taking ownership of a local territory.

You will expand our referral network, promote our full range of clinical services, and work with leadership to implement marketing and growth strategies.

• Maintain referral source activity and account data in the CRM.
• Manage a structured territory plan with regular visits to referral sources.
• Develop and execute quarterly sales and growth strategies.

What You’ll Do

As our Account Executive, you will drive growth by expanding Focus Home Health’s referral network and building trusted partnerships with physicians, hospitals, skilled nursing facilities, and community providers throughout the San Jose area. This role is ideal for someone based in San Jose who enjoys being out in the community, building face-to-face relationships, and taking full ownership of their local territory.

You will:

• Conduct in-services, presentations, and educational meetings with referral partners
• Build and maintain strong relationships with physicians, hospitals, SNFs, ALFs, and community providers
• Promote Focus Home Health’s full range of clinical services
• Work closely with agency leadership to execute marketing and growth strategies
• Maintain referral source activity and account data in our CRM
• Manage a structured territory plan with regular referral source visits
• Develop and execute quarterly sales and growth strategies
• Identify new business opportunities and market trends

Who You Are

You’re a confident communicator who knows how to open doors and build lasting partnerships. You’re comfortable working independently, managing a territory, and driving results in a competitive healthcare market. You bring:

• Excellent communication and relationship-building skills
• A positive, professional, and customer-focused mindset
• Strong business and problem-solving skills
• A valid driver’s license and auto insurance
• At least 2 years of home health or healthcare marketing experience preferred
• A proven track record of meeting or exceeding sales and referral growth goals
