Job summary

Health Link Home Health & Hospice is actively seeking an Account Manager to cultivate referral relationships across Santa Clara County. This field-based role offers a company vehicle and AI-powered tools to enhance territory routing and referral insights.

You will develop partnerships with hospitals, clinics, and senior living communities, conducting in-person outreach, visits, and presentations to drive growth and improve patient experiences.

Qualifications

• 1+ year of experience in healthcare sales, business development, account management, community relations, or a related healthcare role preferred.
• Strong relationship-building and communication skills.
• Sales-oriented mindset with the motivation to develop new business and grow referral relationships.
• Comfortable conducting in-person outreach and developing new professional relationships.
• Strong follow-up, organization, and time-management skills.
• Self-motivated and comfortable working independently in the field.
• Knowledge of the Santa Clara County healthcare market preferred.
• Existing relationships within the South Bay healthcare community are a plus.
• Home health, hospice, post-acute care, senior care, hospital, or skilled nursing experience preferred.
• Valid California driver’s license and driving record that meets company requirements.

Responsibilities

• Develop and maintain referral relationships throughout assigned areas of Santa Clara County.
• Build relationships with hospitals, skilled nursing facilities, physicians, medical groups, clinics, senior living communities, and other healthcare organizations.
• Conduct consistent in-person outreach, account visits, meetings, presentations, and follow-up.
• Introduce new healthcare partners to Health Link and educate them on our home health services, clinical programs, and capabilities.
• Maintain regular communication with existing referral partners and strengthen those relationships over time.
• Identify new referral opportunities through consistent field outreach and relationship building.
• Follow up with prospective referral partners and maintain consistent account contact
• Execute territory priorities and business development initiatives in partnership with the Director of Business Development.
• Communicate feedback from referral partners, market activity, and potential growth opportunities to the Director of Business Development.
• Collaborate with Intake, Clinical, Operations, and other internal teams to support a strong referral and patient experience.
• Meet established referral, admission, and account growth goals.
• Represent Health Link professionally throughout the South Bay healthcare community

The Role

For over 20 years, Health Link Home Health & Hospice has served patients throughout the San Francisco Bay Area. We are seeking a motivated, relationship-driven Account Manager to join our Community Relations team and support continued growth throughout Santa Clara County.

Working closely with the Director of Business Development, the Account Manager will focus on developing and maintaining relationships with healthcare providers, increasing awareness of Health Link’s home health services, and generating new referral opportunities throughout the South Bay.

This is a field-based role for someone who enjoys building relationships, connecting with healthcare professionals, and developing business within the community. We are looking for someone who is motivated, consistent, organized, and comfortable being accountable for growth goals.

You will be supported with a company vehicle, ongoing training, and AI-powered technology designed to provide referral insights and support efficient territory routing.

What You’ll Do
