Job summary

Adult & Child Health in Indianapolis seeks an Access Screener to help transition clients into System services and guide referrals to appropriate providers. You will collect demographics, insurance information, and service needs, ensuring accurate data entry in the EHR and timely follow-up.

The role requires a Bachelor’s degree in Health and Human Services or related field, strong communication, and a customer-service mindset.

Qualifications

• Bachelor’s degree required.
• Experience in client intake or screening preferred.
• Strong communication and data entry skills required.

Responsibilities

• Obtain information to determine if System has appropriate services for client/patient.
• Gather information regarding service needs, demographic, insurance and/or payer information.
• Represent the System and its values in a positive manner; be friendly, welcoming, and approachable.
• Collaborate with community partners and internal service lines to link clients with services via phone and email.
• Complete required documentation and data entry in the EHR and tracking tools.
• Monitor referrals and ensure timelines are met for follow-up.
• Meet established performance metrics: calls returned by 4:00 p.m., collaborative documentation, and referrals documented.
• Other duties as assigned by Program Leadership.
