Job summary

Lutheran Community Services NW is seeking an Intake/Access Coordinator to support individuals seeking behavioral health services. You will coordinate intake, gather information, and conduct screenings to guide clients to the right programs, with emphasis on culturally sensitive care.

HOW YOU WILL MAKE AN IMPACT:

The Intake/Access Coordinator plays a crucial role in facilitating the intake and assessment process for individuals seeking multicultural counseling, family and community support services. This position requires excellent organizational and interpersonal skills to effectively coordinate and manage the intake procedures. The Intake Coordinator serves as the initial point of contact for clients, gathering essential information, conducting screenings, and ensuring a smooth transition into the appropriate programs or services. Addictions PSS, CADC, or CRM, and QMHA or QMHA-R with the ability to obtain QMHA within 30 days is required.

• Serve as the first point of contact for individuals seeking services, either through phone calls, emails, referrals, or in-person interactions.
• Conduct initial mental health and substance use screenings to gather necessary information about the client's needs, history, and current mental health status.
• Triage based on the client's situation to determine appropriate services or level of care.
• Track and monitor student grades and attendance through regular contact with families and teachers/school counselors.
• Collaborate with clinicians, chemical dependency counselors, and other professionals to ensure accurate assessment and placement of clients.
• Schedule intake appointments for clients and coordinate their enrollment into behavioral health programs or services.
• Collaborate with internal staff and external providers to coordinate referrals and ensure timely access to appropriate services.
• Maintain accurate and up-to-date records of client information, including demographic data, assessment results, and referral details.
• Communicate with clients regarding appointment reminders, required documentation, and any necessary follow-up information.
• Prepare and maintain intake forms, assessment tools, and other necessary documentation for client records and program compliance.
• Enter client information accurately into the electronic health records (EHR) system or any other designated database
• Generate reports and statistical data related to intake and referral activities, as required by the organization or regulatory agencies.
• Maintain confidentiality and adhere to all relevant privacy regulations, such as HIPAA (Health Insurance Portability and Accountability Act) and 42 CFR Part 2.
• Communicate effectively with clients, families, and referring agencies to address inquiries, provide information, and ensure a positive experience.
• Collaborate with the clinical team to discuss assessment findings, treatment options, and appropriate referrals.
• Stay informed about available behavioral health resources, treatment options, and community programs to provide accurate information and referrals.
• Develop and maintain relationships with community organizations, healthcare providers, and other stakeholders to facilitate seamless referrals and collaborations.
• Maintain compliance with all relevant program/department requirements, including and not limited to adhering to all record-keeping and reporting requirements.
• Model and encourage direct, professional communication among staff members and facilitate development of a positive staff support network.
• Demonstrate awareness and sensitivity in the delivery of client services as it relates to the cultural and socioeconomic characteristics of the populations served.
• Other duties as assigned.

HOW YOU WILL BE A GREAT FIT:

• Excellent communication skills with the ability to read, write, and speak in English. Fluency in the additional following language(s): Arabic, Burmese, Dari, French, Karen, Pashto, Rohingya, Spanish, Somali, Swahili, and Ukrainian preferred.
